Multiscale modelling to assess device-related thrombus risk after left atrial appendage occlusion. 1: Physense, BCN MedTech, DTIC, Universitat Pompeu Fabra, Spain; 2: Aarhus University, Denmark Background. Left atrial appendage occlusion (LAAO) prevents thromboembolic events in atrial fibrillation patients with bleeding risk, but device-related thrombosis (DRT) remains a challenge. Case summary. A 56-year-old man at Aarhus University Hospital underwent LAAO surgery. After being administered 150 mg of Dabigatran 2/24h, no signs of DRT appeared in the post-intervention follow-up scan. A multiscale model integrating hemodynamics, thrombus formation events, and dabigatran revealed low concentrations of fibrin due to the presence of anticoagulant, matching the absence of DRT. Discussion. Low-dose DOAC have emerged as effective strategies to reduce DRT and their modelling allow to explain the absence of DRT in the follow-up angioscan. Take Home Messages. Our high-fidelity model offers mechanistic insight about antithrombotic therapy, which cannot be assessed with flow-based simulations alone. The results presented provide proof-of-concept evidence of the model’s capabilities. | |
